About the Providers
Aegis is a leading multi-specialty clinic dedicated to assisting healthcare providers in managing complex patient populations. These patients are often being seen by multiple specialists across the spectrum of care, presenting unique care challenges for their primary care providers (“PCP”) as they are often living with multiple chronic conditions, prescribed with multiple medications, facing intricate management challenges, or experiencing frequent hospitalizations.
Aegis specializes in addressing the unique needs of these high acuity patients using the unique expertise of its highly qualified pharmacists and pathologists. Working with our partner PCPs, Aegis’s experts provide analysis and care recommendations that, when adopted, reduce adverse drug interactions, unnecessary care, and, ultimately, preventable hospitalizations.
Patient Identification and Engagement
Our process begins with collaboration. Aegis works closely with its PCP partners to establish criteria for identifying and selecting patients who may benefit from our expert consultations. Using these criteria, we then work with our partner physician practices to generate an initial list of potential patients matching these selection criteria. This list is then reviewed with the PCPs to ensure that all complex patients are represented. During this process, our partner PCPs often remove or add patients and provide final approval for Aegis to proceed with its reviews. PCPs also may refer specific patients to be reviewed by express requisition. Often these requisitions can be specific requests regarding patients with recent laboratory results.
Comprehensive Review and Recommendations
For each approved patient, Aegis conducts an extensive review of their medical history and current treatment plan. This process involves:
Accessing patient charts and retrieving additional information through Health Information Exchanges (“HIE”s).
Reviewing medications, diagnoses, and records from other healthcare visits.
Making diagnostic and treatment recommendations tailored to better manage the patient’s care.
Coordinating with other providers and specialists involved in the patient’s care team to ensure a comprehensive and unified approach.
Once our analysis is complete, we compile a detailed summary of our findings and recommendations. Each consultation is documented in the patient’s medical record and the report is transmitted directly to the requesting PCP’s EMR platform, facilitating smooth integration into the care plan, to guide clinical decision-making. Providers may occasionally request additional follow-up reviews for these patients to evaluate the progress of their therapy and ensure optimal care outcomes.
Molecular Pathology Expertise
Our clinic’s services are supervised by a molecular pathologist, allowing us to utilize advanced expertise in diagnostic evaluations. Each consult is individually reviewed and approved by our supervising physician, ensuring the highest standard of accuracy and reliability. We utilize consult codes for pathology (CPT 80503 through 80506) to ensure appropriate billing for our work. Our billing reflects the comprehensive nature of our services, which include the time spent by our clinical team on the following activities:
Pulling and reviewing patient clinical information.
Evaluating existing medications, diagnoses, and care history.
Checking for potential medication interactions.
Developing recommendations for modifying medication therapy.
Identifying appropriate future diagnostic testing and other care management.
Collaborative Practice Model and Team Expertise

Supervising Physician: Our supervising physician, Dr. Kevin Rosenblatt, is a licensed MD, PhD Molecular Pathologist, bringing advanced expertise in diagnostic and molecular pathology to guide our clinical operations and recommendations. Dr. Rosenblatt reviews each case and approves the recommendations contained, ensuring the highest level of accuracy and accountability.
Clinical Pharmacists: Our doctors of pharmacy specialize in medication management for high-risk populations. They are adept at optimizing drug therapies, identifying potential interactions, and recommending adjustments to improve patient outcomes and enhance the management of chronic conditions. All work performed by our clinical pharmacists is conducted under the direct supervision of Dr. Rosenblatt and in strict accordance with the collaborative practice laws and scope of practice of the jurisdictions where services are rendered.
In addition to pathology consultations, our clinical pharmacists collaborate closely with our supervising physician. They review patients’ medication regimens and provide recommendations to optimize drug therapy, improve outcomes, and enhance the management of chronic conditions. This integrated model ensures a high standard of care tailored to the needs of complex patient populations.
